What is Chronic Care Management?

Almost one in three of us — especially as we get older — find ourselves living with some type of “chronic” illness, a disease that we learn to manage so it doesn’t interfere too much with our day-to-day activities. And when the disease does flare up, we can treat the symptoms without too much disruption. The best way to stay ahead of a chronic illness like diabetes, high blood pressure, obesity, and similar diseases is to work with a healthcare provider to develop a plan that will help you manage symptoms and prevent new problems. What are chronic diseases?

Chronic diseases are health issues that last over a long period of time and require you to pay attention: Take your meds, talk to your doctor, and seek medical treatment if things get worse.

Will I benefit from chronic disease care?
